FOLIC ACID substitution

Application: Therapy of folic acid deficiencies that cannot be corrected with diet.
Composition: 1 tablet contains 5.0 mg folic acid.
Other ingredients: lactose monohydrate, talc, cellulose powder, colloidal anhydrous silicon dioxide, magnesium stearate (Ph. Eur.).
Contraindications: Hypersensitivity to the active ingredient or any of the excipients. The increase in reticulocytes caused by folic acid can mask vitamin B12 deficiency. Because of the risk of irreversible neurological disorders, before treating megaloblastic anemia, it must be ensured that this is not due to a vitamin B12 deficiency.
Side effects: In isolated cases, anaphylactic reaction, e.g. B. as erythema, pruritus, bronchospasm, nausea or anaphylactic shock. Gastrointestinal disturbances, insomnia, agitation or depression are rare at very high doses.
Dosage form, package size: 20 tablets (N1), 50 tablets (N2), 100 tablets (N3).

What is folic acid?
Folic acid belongs to the group of B vitamins (vitamin B9). Folic acid is involved in the synthesis of genetic material – human cells cannot divide without folic acid. As a result, it plays an important role in all growth and cell division processes. During pregnancy, folic acid is jointly responsible for the healthy development of the child, especially for the development of the central nervous system.

Increased need during pregnancy
According to the German Society for Nutrition (DGE), the daily folic acid requirement for women is 300 micrograms in total. However, most people take in considerably less folic acid than recommended through normal nutrition: as a study with 1,341 participants as part of the baby care program (2012) of the statutory health insurance companies showed, three quarters of all women do not even reach 70 percent of the target value.

In expectant mothers, the folic acid requirement almost doubles, namely to 550 micrograms per day. Of 21,433 pregnant women who were examined for their folic acid status in the Baby Care program, 94 percent of the daily intake was below the 70 percent limit. Two out of three women did not even get half of the recommended daily requirement.

Folic acid deficiency increases the risk of developmental disorders
Folic acid is important for cell division and growth of the embryo from the day of fertilization. Because: The development of the central nervous system begins soon after conception. The neural tube develops in humans between the 19th and 28th day of pregnancy. The neural tube is a structure that later becomes the baby's central and peripheral nervous system. A sufficient supply of folic acid is therefore crucial in order to reduce the risk of developmental disorders in the unborn child - especially of the spine, cleft lip and palate and damage to the nervous system.

Why is folic acid recommended in the form of a drug?
Folic acid is found, for example, in wheat germ, soybeans, green vegetables and whole grains. However, the human body is less able to utilize folic acid from food than synthetically produced folic acid. In addition, many folic acid compounds are sensitive to light, heat or oxygen. Therefore, when cooking, a large part of the Foleic acid are lost. In order to alleviate the widespread folic acid deficiency, table salt, for example, is often supplemented with synthetic folic acid.

It is often difficult to cover the increased need during pregnancy through food alone. Adequate care can therefore become a problem, especially during pregnancy.

However, sufficient amounts of the vitamin are important for the healthy development and growth of the baby. As a supplement, expectant mothers are therefore recommended to take a folic acid supplement every day.

normal dosage
Medical guidelines therefore recommend that all women who want to have a child take 400 micrograms of folic acid in tablet form every day. This supplementation should be continued at least until the end of the first trimester of pregnancy.

When might a higher folic acid dosage be necessary?
A high-dose intake may be necessary for women with a special risk profile* :
  • if there is a family history of neural tube defects or previous pregnancies
  • with special gene variants that are associated with an increased risk of neural tube defects
  • in diseases that impede the absorption of nutrients in the digestive tract - for example in inflammatory bowel diseases such as Crohn's disease
  • in diabetes
  • when taking certain medications
